Interview a Supported Living Agency
A supported living
agency can give you the services you need to live independently. Before you
choose an agency, make sure they understand what you need, and that they can
provide the services you need to live on your own.
You can start by
saying, “I would like to know more about
your agency. Can we set up a time so I can call or visit and ask you some
questions.”
You can use this
paper to ask them questions and make notes about their answers.

After
you talk to the agency, ask yourself
these questions. This may help you to decide which agency is best for you.
